I'm talking here about a desire for real castration, rather than a theatrical fantasy that gives impetus to a CBT session. There are three very different causes of real castration fantasy.

Expectations of the man in modern society create conflict: no longer the stone age brute, how are you expected to be the 'new man' and yet still satisfy yourself through aggressive penetration of the girl? Taught to have much more respect for women, you now suffer conflict over this brutish act.

Disgust with your genitalia could be an expression of transsexuality.

Anxiety about performance, size, humiliation from bullying over the subject: for some, the cause of such problems becomes the focus of your own ill-will.
